Another vote for the Ameristep Doghouse. Given your criteria it is an excellent choice. The folks above already provided the specs and price so I will not repeat it.

Do not go cheap. I have a couple of cheap ground blinds, and while functional, they have some problems.

First, try and sit in a blind and draw a bow before buying. I have to open the back window on mine. I suspect the outhouse is the same. The doghouse looks great from that perspective.

Also, realize that even staked down the blind will flap a little. Best to get one of material that doesn' t make a lot of noise in a breeze. I have been curious how many deer/hogs/turkey have walked around me due to the wind noise on my blind.
