It can happen with a CF gun easily if you reload your own ammo. It has been done, and the gun had problems. Can you blame the manufacturer?

There's an adjusting screw in the lug by the breech. The rod snaps into it. If it pops out of it the rod can slide out. I dropped it in the snow a couple of times and had to go back and find it.
