Attempting to grow my first food plot this year, small one- 1/2 acre, and this is the soil sample results. Any ides on how much limeand fertilizer I need to put down and what kind of fertilizer. I plan on putting EH Throw N Gro this year. I am included a scan copy of the soil sample. Thanks for the help.

That didn't go as well as I hoped because I had to downsize it. The ph is 5.6 and the acidity is 1.8.

your P and K look good. but you'll probably want to add 1.5 tons of lime and about 60 lbs of actual N per acre

I agree the P & K looks good and 1.5 tons of lime per acre would get you in the ballpark. A word of caution on the Throw and Grow it has alot of annual rye in it. Even though it is an annual if allowed to go to seed will produce again next year and can be hard to get rid of (I made that mistake a few years back and ended up starting over and nuking the plot with roundup twice thst year to get rid of the annual rye as it was competing with my clover). If you are going to go with a no till type of seed I would go with WI No Plow or secret spot XL. If you are giong to plow you might try WI Extreme. (I had a plot with a 5.6 PH so I planted extremewhile giving my lime time to act and it performed very well). This is justa suggestion based on my own experience.
