Now that you mentioned it RH, if the end of the shaft is really off square and the insert is too deep it could set the broadhead off its axis.
That is what I was thinking when I said it. That little green thing that comes with the HIT kit is not an ASD. It merely seats the insert to the correct depth. Maybe you didn't use it correctly and it was not seated properly?? Anyway glad to hear about their service and good luck.
