I don't beleive it's a hernia as I thinka hernia is an opening in the muscle wall where the internal organs protrude thru the opening, right?
I would go with elephantitis.

However, severe inguinal hernias can protrude out into the scrotum.
These are the hernias the doc is looking for when they tell you to turn your head and cough.

The popular choice of elephantitis (lymphatic filiriasis) has one flaw. This is that elephantitis commonly occurs in AFrica and that sure didn't look like a picture of an african landscape to me (Not to mention the presence of white tails in Africa). It is also a bit large to be a varicocele and a hydrocele would not have the dark coloring (also size disparity). My bet is a direct inguinal hernia.
