Invest in a good high quality full containment rest such as the Wisker Bisket. You will solve issue number 1.

I would take the opportunity to get a set of good strings and cables.....I am not a fan of the Fuse strings myself, and I just put on a Winner's Choice string on my Vectrix. If I would have had time I would have ordered from Gibblet, but I only had 3 days to get it fixed.....Long story....
Anyway, I love my WC string and have heard nothing but good about Gibblets....Either way, I change 'em...IMO

Man, that sucks. I hope you get everything straightened out. As the others said, check your nocks, and make sure you have enough space between your loop and release.
Good luck.
Good luck.

Couple of other things...
Make sure you're using the right size biskit for your arrow diameter. I've seen bow wrecks - more than a couple, for sure - caused by biskits pulling arrows off the string during the draw, when the hole was too small for the arrow.
And make sure your rest is mounted far enough forward that the vanes aren't inside the bristles at brace height. I saw a biskit clamp down on a fella's vanes one night and held on so tight the nock pulled out of the arrow when he drew. He didn't notice and triggered the release at exactly the same time everybody on the practice range yelled at him to let down. That was a brand new Hoyt. It's third shot was it's last.

Thanks everyone for the replies.
Phil: Takin her in today actually, i'm a little nervous
Arthur: I checked them ALL today and seem fine, they are the original nocks still that came on my BEMAN'S, about a year old. Also, I just put i new "biscuit" piece on my rest, wore my old on out, and yes it is the correct diameter. My arrows are fairly long and i'm also using 2'' vanes so vane and whisker contact is not the issue at hand.
Greg: I'm still not sure on the exact amout of times this happens, not a lot, but your right, it should NEVER happen and i do need to figure out whats going on here.
MOBOW: WC and Gibblet strings are my top two choices, i need to figure out what i'm going to do, that is, if my bow is fine. Instead of 50-100$ i might be spending 700-800
All i can think of, as some have stated is the nocks, I'm no "bow tech" (no pun intended) but can't see a visible problem. I think this may have been a fluke thing, just bad luck...thinking back, maybe i didn't nock it all the way, i really do not know. Lets just say I hope my bow is fine, and hope i get strings fast. Learn from my own mistakes is all i can do. Thank you all for your imput.

I can't say for sure but my guess is nock pinch.Even with a loop this can be a problem.
I doubt that anything is hurt too bad on the bow.The Hoyt limbs are tough.Double check them anyway.
The most likely problem you will see,ESPECIALLY on a Hoyt is the axles have a good chance of being bent.If you have no way of checking them to see if they are still straight, they are cheap enough to change. That would be my reccomendation.
