Depends where the front leg is, forward, it will almost completely expose the heart. Then ofcourse there are quartering shots...

I shot thru the shoulder blade and hit the bottom of the spine and still penetrated thru the deer with a magnus stinger.

I don't see how the shoulder "blade" could ever cover the heart if you look at the diagram. The shoulder "bone" could cover the heart if the deer is walking or quartering toward you.
