I'm sure you can find several places showing how to tie a traditional loop, if you do a google search. They are very easy, but not the best.
Try tying a Catfish Loop. This will take a bit of practice, but it makes the best loop by far. It won't stretch, can be make to a custom thickness and length, and it can't come off.

I'm not sure how to make one. I have seen some made from serving, but I usually buy mine pre-made. Here is how to tie one anyway:
Remember to fray and burn the ends. Be sure to check it often if this is your first time (even if it isn't). You don't want it to pull out.
