if the arrows are spined right, then it could possibly be a form error if you say you only see it once in a while...

but could very well be a tune issue with the bow..

id look at the spine first and make sure its on...after that tune the setup with a walkback or bareshaft or broadhead tune...then if you cant get it to tune and your spine is good, its likely form...
