One of the best ways to ensure that inserts don't pull out is to clean the inside of the shaft properly. That mens denatured alcohol and QTips.

That being said, the best glue I've found is 2 Part 24 hr epoxy and here's why. If hunting arrows I install the inserts with broadheads attached. After installing I like to spin my arows to make sure the broadheads are aligned with the shaft. If they are not I can turn the insert within the shaft to get a better alignment. Then let them sit for the glue to do it's job.

You can't do this very well with CA glues. True, the epoxy is not as quick setting, but if we're talking about paying attention to details then putting arrows together doesn't have to be a matter of "how fast can we get it done". Same goes for fletching, tuning and many other things in archery.
