Just filled mine out w/ receipt and UPC's........in the mail today.

Too bad it's only $15 max, I'm picking up 3 or 4 more boxes today to dial in a back-up gun.

I just sent in my 3rd rebate, still waiting for the first one which I mailed out about a month ago
