Model 70- Extractor claw D/N engage cartridge, bolt pushed cartridge?

Model 70- Extractor claw D/N engage cartridge, bolt pushed cartridge?
On my model 70 (.30-06) a couple of times the claw extractor D/N engage the cartridge and pushed it into the chamber. What would be the most likely cause? Not enough lube?
