Hate to say it...
But my first impression, given the info you present, is...practice my friend, practice.
The H&R "accuracy" is par for the course, IMHO.
On the Omega...
First, 150 grains of anything is overkill, IMHO.
Second, are you being absolutely consistant with your measurement of the charge weight?
Third, are you being absolutely consistant with how much you compress the load?
Fourth, with both firearms, trigger pull quality, is it there or not?
Fifth, with both firearms, sandbags from a solid bench? Screws tight? sling swivels dragging on recoil? Calm wind?
Lottsa things can mess up "accuracy"............

How big is the 10 ring you keep all your shots in for the shotgun? I really done know much about muzzle loaders so im gonna leave that for someone that knows them better. Ive seen slug guns that only like fast moving slugs or slow moving slugs. My gun only shoots fast moving slugs well. It still only gives me about 2.5in groups at 100 yards. Also it could be you. I know you said you know how to shoot but i know how to shoot and from not shooting for about 4 months my groups went from 1/2in to 1in with my 204 ruger. I know thats not a bad group size but i used to easlily keep all my shots in a 1in circle at 200 yards all day.

If your barrel is heating up that could be the cause of the 3rd flier. The main thing to watch is if the first one hits where you point. It only takes one shot the kill a deer.
