I was told they were all user error.

For the record....my D-loop still won't stay seated.;)

tx.....I've had 4 shop techs tie it on......I've tied three on there myself. It stays put, now.....ONLY because the top know IS secure. The bottom know still spins.....but is kindaa non-factor.

Yes. I use BCY material. Yes....The 4 shop techs and I know how to tie a D-loop....lol.
