that appears to be some sort of extreme hurnia which if the sack would burst cause death to the deer that or it is what we call on the hog farm gut bust where the gut lining becomes weakened and ruptures similar to a hurnia causing the guts to grow in a sack outside the body which can also cause death i hope someone has harvested this deer so it did not go to complete waste
