I work in a gun shop and this is something that you might want tohave a gunsmith look at. Every now and then when the factory is fitting the choke, it can mess up the threading by just a fraction of a fraction of an inch. This causes it to be accurate at a real close range, but be way off the chart further out. It could also be your sights, but it sounds like this is what is wrong...if it is whats wrong, and the gun is reletively new, they should replace the barrel free of charge.
