I would say if you can keep the call in your mouth without a gag reflex you can learn to use it.

Look up Scott Ellis on youtube he has some excellent how to videos and also some great DVD's you can purchase if you want. Shane Simpson also has a some good youtube videos and a web site actually I believe they both have web site

When I first started others told me to use a push pin or box call and not a mouth call but I stuck with it and now it is my favorite call to use and pretty much the only one I use.
