Ron, your last photo is a very good example of a seriously flame cut Breach plug. that is a real valley for pressure to leak. causing more cutting.

I believe that it is very possible that not all breach plugs even from the same manufacturer are equal. I very much doubt that the material they are made from is certified specs.
