I am going to go talk with him about getting a mri on it. If not i will go see another doctor about it.

The reason I ask is that you're average family practitioner may not be able to give the same insight that an orthopedic specialist would be able to and it sounds like that is the type of doctor that you need to be seeing.
The thing that sounds really fishy about this guy is that you have been injured for this long and no MRI, surgery, or physical therapy have been prescribed to give you more insight into the problem. You didn't say anything about physical therapy so I assume nothing has been brought up about it and that is really how you will stop the pain. The first thing that should be done is to find out what the route of the problem is solve that and if there has not been an MRI then there is really no way to know what the extent of your injury really is. Giving you vicotine and cort shots will make you feel better in short run but they don't help you get a rehabilitated state with your shoulder or knee and it sounds like that is what your wanting. So my first suggestion is that you do get the MRI and talk to someone who deals with these types of injuries on a daily basis. If you don't like advice then go get a second opinion.
