I've heard of it and seen pictures but was never around when it happened. And I don't ever want to be. If an arrow has a crack or should be smashed and thrown away. If you hit one with the other, check them out carefully. Any time you miss the target or critter you should double check them. Look your arrows over carefully and maybe even apply a little twisting pressure to them to expose weak areas. There was a picture a couple years ago floating around on these boards where a carbon shatterred and slayed out into several pieces that all pierced the shooters hand like yours. I mean, it was ugly. He took the picture with it still stuck in his hand.
