Yes, I have heard of it happening. That is why it is critical to inspect carbon arrows everytime before you start shooting. I have made it a part of my pre-practice ritual to inspect my arrows. Also if while shooting I hit something solid or hit one shaft with another I will check those arrows out very carefully to make sure they are in good shape and safe to shoot. Another thing to keep in mind is making sure the spine of your arrow is stiff enough for your bow.
