What type of bolt are you shooting? If it’s aluminum it’s possible that it is bent a tad. That would account for you bolt hitting all over the place. Depending on which way you sat it in your rail would determine which direction it flew.
With your broadhead in the bolt, put the tip on a table and spin the bolt. If it wiggles at all, it could be a bent bolt or maybe the broadhead is not centered well. Don’t give up, just eliminate issues and you’ll find the source.
