That looks like it may have been an instance of excessive pressure.

Maybe a reload with a double charge of powder, or a round that had been chambered too many times for duty, eventually forcing the bullet back into the case, reducing the case volumeenough to crank up the pressure beyond a safe level.

Gotta rotate that ammo guys.
After a few times through the chamber, send it downrange.
