Lease Permission Slip

I finnaly found the lease i've wanted. What do you guys use for a permission slip/form. The was a few of them on here a long time ago but cant find them. Any help is appreciated. Thanks!

In MD you have to have written permission to hunt private land. MD's DNR site has a form. MI might have one as well, but here is a link tot he one that MD has.

The land that I hunt the owner makes up laminated cards with a statement that the holder has permission to be on that property for the purpose of hunting/scouting and all other outdoor activity that is appropriate. He includes a start/expiration date as well as his phone # for verification. He also changes the color of the card every year.
