Looks like maybe thats his left antler hanging down...You can kind of see the main beam with the g2 and 3 sticking up. I'd say he broke it and it's still in velvet kind of.

my guess is he was hit by a car. the broken antler looks black from the dried blood. explains the spots on his neck too. cool pic though !
