I have had about 25 emails wanting this so here it is:

Thats a good form. I havent had any trouble with landowners requesting one but if i ever do its nice to know about this one! Ill save it to my computer for the future!

i always recomend using a form just so your butt is covered. might shoot a monster buck and then the land owner might say you didn't have permision. like what happened in oklahoma a few years ago

Both very nice ...the Ohio DNR has one that's printable from thier website. If hunting in Ohio I recommend using the "official" form to avoid any translation errors with Mr. Warden.
