How high should the nocking point on your string be compared to your arrow rest (I shoot a shoot-thru style rest)?

All depends, 3/32 to 3/8 high.

I usually start about 1/4"ish high,but you have totune to find the exact proper location. For future reference, make a note of how high you started, and where it ended up after you tuned. Then you'll know for next time, and you won't have to mess with it as much during tuning.

When you hear guys saying that they eyeball it and it comes out right, that's because they are used to how their bow gets set up and have bothered to remember it. Alwasy check the tune anyway.

Yep that sound's right from 1/4 " to 3/8"

Depends on cam system. One cams I start 1/8" high and go from there. Other dual cams, binaries, or cam and a half's I tend to get either at 90 degrees, or real close to it.
