I don't think this is required by law but Illinois DNR web site has a sample permission slip.

Uncle Matt had emailed me the slip a while back, but i accidentally deleted it.Based on what folks have been saying onhere,I'm not gonna worry about it.

Having written permission from everyone would be a pain in the arse, I wish it was how it used to be where everyone just shared the land. I can see the problem with deer hunting, but coon hunting should be open land access.
