nock and insert issues
 

when i was shooting yesterday i had a issue with one of my nocks pulling out. had this happen before one time i shot a arrow and the nock stayed on the string.
I also had a field point and insert pull out when i went to remove a arrow from my target which left it in the target
how can i solve that can i just glue the inserts in place.
I have also had issues with the insert turning when i change tips.
