My only suggestions is that instead of saying "Permission is granted for: (check)" I would say Permession is granted for all of the activities listed below except those which are checked.

It will make the form easier to fill out, harder to doctor, and I think IMHO that you will get more permitted activities in the long run.

Using a form like this is great as it eliminates misunderstandings. Nice job.

Great post! I have one comment and a question.

First, I am wondering how valid liability laws are in the event of death, such as will the laws in most states still protect the landowner in the event of a hunter's death if the hunter's survivors try to take action against the landowner? This is the only reason a particular landowner I hunt on will not grant written permission, just says go ahead but will deny I have permission if something would happen to me.

Second, would it be a good idea to have the permission slip notorized?

I think the state in which you reside or hunting in makes the difference. I know here in TX this form will hold up to protect both parties.
