Strange thing happen in the wild and I agree the shadows and all look good. So I don't beleive the picture was doctored with shop. Not saying the antler wasn't drove into the neck after the deer was dead so only the person that shot it or was present when recovered could honestly know if the added antler was fakedly put into the deer.

I saw this last year and enlarged it 500% and could not see any "jaggies" or other signs of doctoring. I say real.
