I start at the top of the serving (top limb)
Start with whats called a Jam "knot". Make a wrap, then start wrapping down over the tag end, pretty simple. I go a good 10 or 12 tight wraps, then pull the tag end tight and trim. If you are a little small on the serving diameter, leave the tag end to increase the size.
The tying off is harder to explain then it is to do.
The knot is whats known as an Inline Whip Finish. Very similar to a whip finish in fly fishing, however the execution is different.

I personally DONT cut the material off my bobbin here, I prefer to use it for weight and to back wrap snugly to keep things from coming unglued should a finger slip. Pull out a enough serving so you can counter wrap (back towards the top limb) with the serving tool (bobbin) still attached. I go 10-12 turns here. Lay your bobbin end of the thread over your initial wrap before the loop is made and let go of the bobbin holding onto the loop. What you are going to do here is wrap as you started off (same direction). What happens is you'll unwrap those last 10 or 12 counter wraps you've done. The bobbin end will slid letting you tighten by pulling on the bobbin, trim, burn the tag if you like, and you're done.
