Slick Trick broadhead failure???

Hey guys I posted this on the bowhunting site but I wanted to get the word out to the techs - can you look at this post

and give me your input - from the tech side - i.e. do you think the head design could be an issue?? I know some of you don't regularily vistit the bowhunting page & I'd like your input too....
