Default jammed bolt

Are there any gun experts who can help me? The bolt sleeve on my 7mm mag turned while I was loading and prevented the bolt from sliding back into firing position. This cost me a nice mulie and I don' t want it to happen again. Has this happened to anyone else? What caused this to happen?

What make and model of gun.?

The gun is a Winchester model 70 7mm mag.
